Please complete the relevant forms as outlined below for Laboratory Use and submit them to our Faculty Dean's Office via the Department Chair.
All undergraduate and graduate students who will work in our faculty laboratories, as well as all researchers from outside the faculty, must declare that they have read and understood all the rules listed in the "LABORATORY SAFETY AND WORKING RULES GUIDE."
Researchers who are not academic staff members of our faculty must complete the “Faculty of Science Laboratory Waiver Form (Lab. Form A)” and submit it to the responsible person of the laboratory with whom they wish to collaborate. The responsible laboratory staff must then prepare and approve the “Laboratory Work Permit Certificate (Lab. Form B),” with the waiver form attached, and submit it to the Dean's Office through the relevant Department Chair. Work may only commence after the Dean's approval is granted. All laboratory use requests received via our university’s Laboratory Information System (Labsis) for our faculty laboratories will be evaluated according to the procedure described in this section.
Students who participate in laboratory sessions during working hours must complete and sign the “Student Risk, Waiver, and Consent Form (Working Hours) (Lab. Form C)” at the beginning of the semester and submit it to the relevant Department Chair. After this, students may carry out work in laboratories under the supervision of the designated laboratory responsible. Once collected, a copy of these forms must be submitted to the Dean's Office by the Department Chairs.
Students who wish to work in laboratories outside of regular working hours must fill in the “Student Risk, Waiver, and Consent Form (After Hours) (Lab. Form D)” and obtain the approval of the responsible laboratory staff. The form must then be submitted to the Dean’s Office through the relevant Department Chair for approval. Work can begin only after the Dean's approval is granted.
